We accept dialysis patients who are visiting from other areas for travel or work.
Please make a reservation and have your primary care physician contact us at least two weeks before your desired dialysis date.

How to Make a Reservation
1)Reservation via Web Form
Please fill out the Temporary Dialysis Reservation Form available on our website.
Please make your request at least two weeks before your desired dialysis date.
We ask you to provide the following details
- Patient’s full name
- Preferred date
- Desired time slot (morning or afternoon)
- Address and contact email
- Contact number
- Primary care physician’s name
- Presence or absence of infectious diseases
If you wish to use a changing room locker, please let us know.
Please note that due to infection control measures or bed availability, we may not be able to accept your request.
2)From Your Primary Care Physician
Please ask your doctor to send the following documents via FAX at least two weeks in advance

- Medical information summary (please also bring the original on the day)
- Dialysis condition sheet
- Dialysis progress reports from the last three sessions
- Blood test results (including infectious disease information)
On the Day of Your Visit
Please come to the 1st floor reception.
You will then be guided to the 3rd floor dialysis room.
If you have any symptoms such as fever, cough, diarrhea, or vomiting, please call us before your visit.
Payment
Please make your payment at the 1st floor payment counter.
Items to Bring
- Health insurance card
- Specific Disease Medical Certificate (if applicable)
- Disability certificate (if applicable)
- Original medical information summary
- Referral letter from your primary care physician
- Non-woven mask (must be worn at all times in the hospital)
- Earphones (for TV viewing)
- Other usual dialysis items (e.g., hemostatic belt, regular medication, towels)
Notes
Meals, transportation, and hospital gowns are not provided for temporary dialysis patients.
If you lose the changing room locker key, a replacement fee of 1,100 yen will be charged.
